PURPOSE: To recognize fiddlers throughout New York State who have contributed to the preservation,
perpetuation, and promotion of the tradition of Old Tyme Fiddling.

I/We believe this individual meets the following criteria for nomination:

1.    An advanced fiddler who is known to have contributed to the preservation, perpetuation, and promotion of the
       tradition of Old Tyme Fiddling;
2.    A New York State recognized fiddler;
3.    May be living or deceased;
4.    Involved in performing and/or teaching activities;
5.    Inductee need not be a member of the NYSOTFA. 

I/We are submitting the following items:

[   ]Biography of approximately 200-250 words including history of the nominee, music experience, examples
             of how the nominee meets the above criteria (please use reverse side if additional space is needed):

Please note, nominees shall not be automatically carried over to the next year and must be renominated; any
new support items must accompany the new nomination form.  Nominations must be made on the NYSOTFA-
authorized Nomination Form and Emailed to Jackie Hobbs at jmhobbs@twcny.rr.com, or mailed via U.S.Mail, postmarked by September 1 and send to:
